Player Insurance

As part of a participant’s membership, Futsal HQ provides low-level Personal Accident Coverage (PAC) for registered participants* within our competitions.

*Definition of registered participant : A registered participant is an individual who is engaged in activities that are provided, organised, supported or sanctioned by FutsalHQ with the individual’s personal details provided in their entirety on a validly lodged Registration Form prior to participation.

Many recreational activities involves a degree of physical risk and participants may sustain personal injuries or death as a result of my participation in recreational activities. Injuries which may occur on premises under the control of Futsal HQ include (but are not limited to) the following types of injuries: injuries that may occur as a result of flooring, a futsal ball striking a player, collisions with other players of the premises or injuries relating to or caused by equipment. In order to keep competition costs at modest levels, players are encouraged to arrange their own separate insurance arrangements against the event that they injure themselves during the course of playing. All participants must consider medical, social, paramedical costs as well as their own financial position if they are unable to work for an extended period before participating in a Futsal HQ competition. Players are therefore encouraged to insure themselves get their own personal medical and ambulance cover and to ensure adequate income protection insurance.

The PAC available through Futsal HQ is not a substitute for Private Health Insurance. Additionally, the coverage does not provide for loss of income or replacement for salary and wage earners. We refer you to the Personal Accident Policy Schedule that is to be perused in conjunction with the Policy Wording where you can make a valued judgement on the level of cover provided and suitability to your circumstances.

The Health Insurance Act (Cth) 1973 does not permit the insurer to contribute to any charges covered by Medicare (including the Medicare Gap).

This means that any charges for treatment from a doctor, surgeon, anaesthetist or surgeon’s assistant are not covered. It also means charges for x-ray, some MRI scans (if Medicare registered) and public hospitals are not covered. In addition, there will be no refund in respect of:

  1. Any expenses recoverable by you from any other insurance scheme or plan providing medical or similar coverage, or from any other source except for the excess of the amount recoverable from such other policies/plans.
  2. Any expense to which the National Health Act (Cth) 1953 or any of the regulations made there under apply.

*MRI Scans are generally covered through Medicare; however please check with your treating physician as sometimes the provider is not registered with Medicare.

All competitors should be sufficiently fit, medically cleared and ready for competition to maximise enjoyment and to minimise personal injury. If unsure, please consult your Medical Practitioner before commencing in competitions offered by Futsal HQ.

  • AN EXCESS OF $800 APPLIES TO EACH CLAIM THROUGH THIS POLICY (INVOICE PAYABLE).

Futsal HQ was officially born in 2015 by sports administration & futsal enthusiasts in order to create fitness and socialisation opportunities for members of the community, particularly in Melbourne’s inner-south-eastern suburbs.
